Staff and Non-Physician Provider Benefits

The Oregon Clinic provides a comprehensive benefits package that becomes effective the first of the month following 30 days of employment. Programs include the following:

Medical, Vision, and Rx Insurance — employee coverage is 100% paid for by The Oregon Clinic for full-time employees (prorated credit given for half-time employees) Additional medical plan options are available for employee’s qualified dependents. The Oregon Clinic provides a dependent subsidy towards your family’s medical premiums of up to $120.00 per month for full time employees (prorated for half time employees)

TOC offers a self-insured PPO plan administered by Regence BlueCross BlueShield of Oregon. This PPO plan allows individuals to access care without requiring a referral and utilizes the preferred and participating panel of providers through Regence BlueCross BlueShield of Oregon. The Core plan has an annual $400.00 deductible (per covered individual) that is waived for office visits and preventative care. Office visits at preferred and participating providers have a $15.00 co-pay for primary care and $25.00 co-pay for specialty visits. Preventative care is covered at 100% at preferred and participating providers. The out-of-pocket maximum for in-network benefits is $3,750 (per covered individual). Out out-of-network benefits have a $550 deductible (per individual) and then generally services are covered at 70%, with an out-of-pocket maximum of $4,400. The Core plan also includes an annual vision exam and provides a $200 allowance for hardware every 12 months. There is also a 3-tier prescription drug benefit included in this plan.*

Dental Insurance — employee coverage is 100% paid for by The Oregon Clinic for full-time employees (prorated credit given for part-time employees)

TOC offers the choice of two dental plans:

Regence PPO Plan: a traditional 100/80/50 plan covering 100% of for preventive care, 80% of basic services and 50% of major services. There is a $50 deductible, waived for preventive services and $1,500 calendar year maximum. The plan allows individuals to seek care from any dentist, however out of network services are subject to UCR and balance billing.*

Willamette Dental: fee for service plan, $15.00 co-pay for preventative care, $15-$250 co-pay for basic services and $75-$500 co-pay for major services. Orthodontia $2,500 co-pay covered for Adults and children. Members must use a participating Willamette dental provider; service rendered by a non-Willamette dental provider will not be covered.*

Life Insurance — base policy 100% paid for by The Oregon Clinic

The Life and Accidental Death and Dismemberment plan provides one times an employee’s annual salary, up to a maximum of $50,000 of coverage. Supplemental coverage is available on an individual basis with through Standard Insurance, with a guaranteed issue of $100,000 if elected at initial enrollment.*

Long-Term Disability — 100% paid for by The Oregon Clinic

The long-term disability plan for staff covers 60% of monthly salary to a maximum of $7,500 per month after 90 day elimination period and benefits are payable until retirement age.*

Voluntary Short-Term Disability

The plan provides coverage of 60% of weekly salary, up to a maximum of $2,000 in weekly benefit. Plan includes a 14 day elimination period and benefits are payable up to 90 days to coordinate with the LTD plan.  Premiums are made though a post-tax payroll deduction.*

Retirement Plan:

401K

  • Eligible to participate the first of the month following three months of employment. 
  • Pre-tax salary deferrals up to 75% of monthly compensation.
  • TOC matches employee deferrals dollar for dollar, up to a maximum of 5% of employee compensation (up to the IRS wage limits).
  • 6 year vesting schedule for employer matching contributions.

Profit Sharing

  • Eligible to participate at three months of employment. 
  • Fully funded by TOC, 3% of participants eligible income.
  • Must be employed on December 31st to receive to contribution for that year.
  • 100% vested immediately.
  • Plan discontinues once participant qualifies for the Fixed Contribution Plan.

Fixed Contribution

  • Must have completed 2 years of employment.
  • Enrollment is semi-annual (Jan. 1 and July 1).
  • Fully funded by TOC, 5.7% of participants eligible income up to the social security wage maximum and 11.4% of participant’s income between IRS wage maximum and social security maximum
  • 100% vested immediately.
